Privileges
Thank you for your interest in joining the Community Memorial Healthcare Medical Staff.
To begin the process, please send an e-mail to our Medical Staff Credentials Coordinator at credentials@mycmh.org, and include the following information:
- Your full name
- Your street address, city, state & zip
- Your phone number
- Your medical specialty
- The name of any existing medical group or local office practice you anticipate joining
- Allied Health Practitioners – please include the name(s) of your supervising physician(s)
Once the e-mail request is received, the Medical Staff Office will respond to you with a link to our medical staff application, specialty privilege forms, and instructions on how to complete the process. Next our Medical Staff Credentials Coordinator will review your submission and determine if you meet qualifications for appointment as outlined in the Medical Staff Bylaws.
