As a patient of our Community Memorial Health Centers, you have access to convenient care throughout west Ventura County through a comprehensive network of primary and specialty care services. Our expert providers offer in-person and virtual appointments and we have several Urgent Care locations with extended hours.

MyChart is Community Memorial's NEW, easy-to-use patient portal. It lets you and your providers see all your health information in one place - across our Hospitals, Health Centers, pharmacy, lab, and Continuing Care Center. Patient Forms

Prepare for your upcoming appointment by completing your patient forms and questionnaires online. Additional documents may be required at the time of your visit, but completing these forms in advance will help us better prepare for your appointment.

After-Hours Care

Beginning June 1, 2026, getting care for you and your loved ones will be easier to fit into your schedule. We are proud to announce the addition of after-hours primary care appointments at select Community Memorial Health Center locations throughout west Ventura County.

After-Hours Care offers scheduled family medicine and pediatric appointments outside of traditional business hours. It is designed for people who need primary care appointments that better fit work, school, and family schedules. Click here to learn more or call 805-948-6353 to schedule.

Chronic Care Management Program

Sometimes, Community Memorial Health Center patients with chronic illnesses need a little support managing their care. It can be overwhelming to understand medications, schedule and track visits with specialists, and coordinate care on their own. For these patients, Community Memorial’s Chronic Care Management team is here to help.

Caregiver Navigator Program

Caring for an ill parent, child, spouse, friend or loved one can be stressful and overwhelming. Our Caregiver Navigator program is a free program that provides resources and support to those caring for patients with complex medical illnesses, mental health issues, Alzheimer’s/dementia, and more. The program ensures that caregivers don’t have to manage the care of their loved ones alone.

Dementia Care Management Program

Dementia Care Program (DCM) is a care management program tailored for patients diagnosed with Alzheimer's or any other forms of dementia. Based on UCLA Alzheimer’s and Dementia Care (ADC) Program, Community Memorial’s DCM program supports patients and families in navigating the intricate medical, behavioral, and social aspects of dementia. Led by our Advanced Practice Dementia Care Specialist (DCS), we offer comprehensive and collaborative dementia care within our Health Centers.

Diabetes Self-Management Education

Our Diabetes Self-Management Education (DSME) program gives diabetes patients the knowledge, skills, and support to manage their diabetes with confidence. Our Diabetes Care & Education Specialists work with patients to design management plans tailored to their lifestyle, culture, beliefs, and unique care needs.

Enhanced Care Management Program

The Enhanced Care Management (ECM) program provides extra care coordination for Gold Coast/Medi-Cal members with complex health and social needs. This program can help patients better understand their care, connect with services, and feel more supported as they manage their health.

Substance Use Navigation

Our Substance Use Navigator program provides compassionate, judgment-free support for people who are struggling with alcohol, opioids, or other substances, helping them understand their condition, start treatment, and connect with follow-up care. At Community Memorial, Substance Use Navigators work collaboratively with our Addiction Medicine specialists to help patients access medical detox, outpatient support, medication-assisted treatment options, support groups and more through our Addiction Medicine .

Workplace Health

Community Memorial Workplace Health partners with employers to meet the unique healthcare needs of a business, and ensure that their team members receive the quality medical care they need. Our program focuses on the screening, prevention, treatment and monitoring of injuries and illnesses sustained by employees at work.
