Powered by HealthAware, our online risk assessments are designed to help you determine your risk level, as well as connect you to resources and experts who will guide you in the process of lowering your risk and improving your overall health.

Each free assessment consists of a confidential questionnaire that takes about 7 minutes to complete. Once you’ve finished the assessment, you’ll receive a personalized report explaining where you have positive risk factors and what each one means.

Depending on your risk level, you may be offered the opportunity to connect with a member of our Wellness team to discuss the resources and support we have available.. Community Memorial offers a variety of programs and services designed to help you make positive changes to your lifestyle and lower your overall risk for chronic disease, diabetes, heart attack, and stroke.
We can also work to connect you to a primary or specialty care physician for evaluation and medical care, if needed.
