Our experienced team of professionals includes registered nurses and dieticians, personal trainers, fitness instructors, certified educators, and community health workers. All of them can play a part in helping you make the most of our wellness resources and guiding you in the pursuit of your personal health goals.
Our team also includes an expansive network of Community Memorial primary and specialty care physicians. Your physician may refer you to our Wellness program for guidance with certain aspects of your health, or we may help connect you to a physician when you need care that goes beyond what our wellness program offers.

Powered by HealthAware, our free online health risk assessments are an excellent way to understand your risk for chronic illness such as diabetes or heart disease, or acute health incidents such as heart attack or stroke. Assessments are also available to evaluate your stress level, lung health, and body weight. Each assessment takes less than seven minutes, and upon completion, you’ll be given the opportunity to connect with a member of our wellness team for more information.
Conveniently located on the campus of Community Memorial Hospital in mid-town Ventura, our Wellness & Fitness Center is a full-service gym and so much more. As the hub of our wellness program, the Wellness & Fitness Center offers group exercise classes, fitness assessments and personal training, blood pressure, blood glucose, and cholesterol screenings, body composition analyses, nutrition coaching and health education classes.
Community Memorial’s registered dieticians are experts at helping you understand the role food plays in your overall health. Our outpatient nutrition coaching services are available in-person or via telehealth.
Having an established relationship with a primary care physician is a vital component of your wellness program. Community Memorial offers a network of family, internal, geriatric and pediatric physicians conveniently located across west Ventura County.
