Each free assessment consists of a confidential questionnaire that takes about 7 minutes to complete. Once you’ve finished the assessment, you’ll receive a personalized report explaining where you have positive risk factors and what each one means.

Depending on your risk level, you may be offered the opportunity to connect with a member of our Wellness team to discuss the resources and support we have available..
