About This Location
The Community Memorial Continuing Care Center is a 75-bed patient/resident oriented facility for both short-term and long-term placement located on the campus of Community Memorial Hospital ― Ojai. As the only skilled nursing and rehabilitation facility in Ventura County located on a hospital campus, the center provides easy access to acute and emergency care when needed.
All physicians practicing at the Continuing Care Center are part of the Community Memorial Medical Staff. The specialized nursing team is on-site 24/7. Oversight is provided by a medical director. This facility offers:
- Post-surgical care
- Short-term skilled nursing & rehabilitation for persons recovering from surgery, a serious injury, or illness
- Long-term, custodial care for certain health conditions
- Hospice and palliative care
- Inpatient and outpatient physical, occupational, and speech therapy services
- On-site imaging, phlebotomy, and lab services
