National Healthcare Decisions Day is approaching on April 16 and serves as an important reminder for all of us to consider and discuss our healthcare preferences and advance care planning.

Dr. Jim Hornstein along with Erin Sowell, RN, will present how to complete your advance healthcare directive and POLST forms. They will also discuss the benefits for both patients and healthcare providers.

We encourage everyone to attend this informative session.

Check-in is at 3:00 pm, the presentation begins at 4:00 pm, and a Q&A session will be held at 5:00 pm.
