Trauma, Burn, and Emergency Surgery PGY4

EDUCATIONAL GOALS & OBJECTIVES

PGY4 resident who complete an elective rotation focused on Trauma, Burn, and Emergency Surgery will gain a strong understanding of how to assess and manage Trauma, Burn, Emergency Surgery patients.

Faculty will facilitate learning in the 6 core competencies as follows:

Patient Care

I. Resident is the team leader for trauma resuscitation, and should implement the trauma resuscitation guidelines

II. Manage fluid resuscitation of each patient and identify and correct coagulopathy

III. Gain Experience and build competency in the performance of urgent and emergent surgeries, emergent procedures and urgent ICU related procedures:

  • Exploratory Laparotomy
  • Diagnostic Laparoscopy
  • Damage Control Laparotomy
  • Hepatic Packing for Trauma
  • Small Bowel Resection and Anastomosis
  • Small Bowel Repair for Trauma
  • Large Bowel Resection, Anastomosis, or Diversion
  • Drainage of intra-abdominal abscess, complex
  • Pancreatic Debridement or drainage for trauma
  • Splenectomy, open for trauma
  • Tracheostomy
  • Percutaneous Tracheostomy
  • Emergent cricothyroidotomy
  • Emergent thoracotomy
  • Soft tissue infection/abscess, complex

IV. Resident should be proficient in the following technical skills:

  • Perform open diagnostic peritoneal lavage
  • Discuss and demonstrate cricothyroidotomy
  • Discuss and/or demonstrate emergent thoracotomy, aortic occlusion, pericardotomy

V. Able to direct the continued resuscitation of the critically ill trauma or emergency surgery patients which includes coordination of consult services, direction of junior residents, and continued evaluation of the patient

VI. Resident should be able to independently identify deterioration in a patient’s status and be able to develop a plan of intervention that will be discussed with the attending staff

VII. Assess the burn wound's appearance with regard to its depth, microbial status, potential for healing, and need for intervention

VIII. Conduct burn wound debridement and prepare it for skin grafting for burn patients

IX. Participate in the acute resuscitation and care of the burned patient

Medical Knowledge

I. Gain understanding of the anatomy, physiology, and function of organs and organ systems affected by traumatic conditions and operative procedures

II. Discuss the techniques of resuscitation of traumatic conditions and operative procedures and relationship to mortality, morbidity, and course of patient recovery

III. Teach the anatomy, physiology, and function of organs and organ system affected by traumatic surgical conditions and operative procedures

IV. Familiar with various disease processes affecting organ systems commonly encountered in trauma and emergency surgical patients

V. Acquire a comprehensive understanding of the required follow up for various trauma and emergency surgical procedures, and provide informed recommendations accordingly

Practice Based Learning

I. Utilizes information technology to prepare for surgical procedures, bringing current modalities of care and the scientific evidence supporting them to the operating room

II. Regularly assesses the efficacy of own practices in caring for critically ill trauma or emergency surgery patients

III. Enhances personal practices in trauma and emerge patient care by integrating well-collected data and feedback

IV. Residents shall keep logs of their operative cases and all procedures and track their operative proficiency as gauged by whether they assisted or were the surgeon junior or senior or teaching assistant

V. Resident shall also keep a log of all the non-operative trauma cases admitted to ICU in which they were the team leader

Interpersonal and Communication Skills

I. Educates patients and families in post-operative and rehabilitative strategies for critically ill trauma or emergency surgery

II. Provides adequate counseling and informed consent to patients

III. Resident will work effectively as part of the trauma and emergency surgery team

Professionalism

I. Cultivates an awareness of the unique challenges faced by families caring for critically ill trauma or emergency surgery patients

II. Exhibits a strong commitment to a set of moral and ethical principles

III. Honors and effectively collaborates with other members of the critically ill trauma or emergency surgery team

IV. Offers timely consultations when requested

V. Maintains reliability, punctuality, and accountability for one's actions in both the operating room and clinic

VI. Safeguards patient confidentiality

VII. Maintain high ethical standards in dealing with patients, family members, patient data and other members of the healthcare team

System-Based Practice

I. Coordinates aspects of the rehabilitation of the critically ill trauma or emergency surgery patient

II. Facilitates the timely discharge of critically ill trauma or emergency surgery patients

III. Practice high quality cost effective patient care
